Res Hall PHE Program Reporting Form

Please submit the following form whenever you complete a program in your residence hall. Remember that as a Res Hall PHE you are required to complete:

• A minimum of three topic-based programs per semester (one program in September, one in October, one in November). Two of the programs must be active and one may be passive.
• A minimum of four outreach and programming aimed at increasing your visibility within your hall.

Please note that your submission of this form allows us to keep track of your fulfillment of your PHE requirements.
